Officers from the Lakewood Township Police Department responded to the intersection of Madison Avenue and Courtney Road at approximately 8:15 a.m. on Wednesday, March 9, 2022, for a report of a significant motor vehicle collision resulting in a fatality. The Ocean County Prosecutor's Office Vehicular Homicide Unit and the Lakewood Township Police Department Traffic Safety Unit initially determined that Huerta-Arias was driving northbound on Madison Avenue in a 2012 Infinity G35 when he collided with a 2008 Chevrolet Impala driven by Mr. Mandel, who was attempting to make a left-hand turn onto Courtney Road. Mr. Mandel's car was thrown into a traffic signal as a result of the collision. At the scene, Mr. Mandel was pronounced dead. Huerta-Arias was sent to Jersey Shore University Medical Center for treatment.

Further investigation by the Ocean County Prosecutor's Office Vehicular Homicide Unit and the Lakewood Township Police Department Traffic Safety Unit revealed that Huerta-Arias' vehicle was traveling at an average speed of 91 miles per hour at the time of the crash, which was more than double the 40 mile per hour speed limit posted on the road. As a result, Huerta-Arias was charged with Vehicular Homicide on June 7, 2022, and he surrendered on the same day at Lakewood Township Police Headquarters. Huerta-Arias was taken to the Ocean County Jail and is currently being held there pending a detention hearing.
